Background:

FamilyWell is the first behavioral health start-up to implement a perinatal collaborative care model that embeds a wrap-around mental health program directly into OB/GYN practices for pregnant and postpartum individuals. Our mission is to solve the maternal mental health crisis by increasing access to equitable, affordable, and accessible mental health care. Our specialized care team consists of social workers, coaches, therapists, and psychiatric providers in the OB provider’s clinical workflow (both in-person and virtually), ultimately becoming the behavioral health arm of the OB/GYN practice.

Role:

The Perinatal Behavioral Health Coach (PBHC) is a core member of the collaborative care team, which also includes OB providers, Behavioral Care Managers (BCM), Licensed Therapists, and Psychiatric Consultants. The PBHC is responsible for providing virtual coaching sessions for patients in the collaborative care program with mild symptoms of a Perinatal Mood and Anxiety Disorder (PMAD). The Perinatal Behavioral Health Coach collaborates with the BCM and Psychiatric Consultant to determine clinical management recommendations when appropriate.

*All duties are virtual and there is no in-person requirement.

Requirements:

  • Qualified applicants must have at least 1 year of coaching experience or experience working directly with perinatal patients.

  • Qualified applicants must have formal education in social work, nursing, or psychology, with a minimum of a bachelor or masters degree in one of these fields.

  • Qualified applicants do NOT need to be licensed mental health providers.

  • Completion of a background check.

  • Competition of the FamilyWell Perinatal Behavioral Health Coaching Certification Program within 3 months of contractor’s start date.

  • Passionate, driven, & inspired by a mission to improve perinatal mental healthcare, particularly in vulnerable patient populations.

Responsibilities:

All responsibilities are performed virtually by tele-health. There are no in-person requirements and the PBHC may work exclusively from home.

  • Support the mental health of pregnant and postpartum patients via telehealth for an assigned patient caseload.

  • Communicate closely with the collaborative care team.

  • Provide patient education about common perinatal mental health disorders and the available treatment options.

  • Provide or facilitate internal or outside referrals to evidence-based psychotherapy or psychiatric treatments.

  • Document patient progress and client encounters in a HIPAA-compliant electronic medical record system and client registry provided by FamilyWell. 

  • Communicate patient progress with medical providers, Psychiatric Consultants, BCMs, and other FamilyWell team members.

  • Identify patients who are not improving and may need more intensive mental health care and report any concerns to the medical provider, BCM, and the Psychiatric Consultant.

  • Facilitate referrals for clinically indicated services outside of the organization under the supervision of the BCM (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

  • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the program.

  • Report to the Lead Coach for the collaborative care team.

  • Prioritize FamilyWell’s mission and vision in speaking engagements, social media, and other public engagements as needed.
