Background: 

FamilyWell is the first behavioral health startup to implement a perinatal collaborative care model that embeds a wrap-around mental health program directly into OB/GYN practices for pregnant and postpartum individuals. Our mission is to solve the maternal mental health crisis by increasing access to equitable, affordable, and accessible mental health care. Our specialized care team consists of social workers, coaches, therapists, and psychiatric providers in the OB provider’s clinical workflow (both in-person and virtually), ultimately becoming the behavioral health arm of the OB/GYN practice. 

Role:

The Behavioral Care Manager (BCM) is a core member of the collaborative care team and plays a pivotal role in supporting pregnant and postpartum patients, including those who have experienced loss, within OB clinics. This role involves educating, recruiting, enrolling, and supporting eligible patients in FamilyWell’s perinatal collaborative care program and ensuring high engagement and satisfaction with the program. The BCM will also coordinate care, track patient outcomes, provide direct clinical support in the form of intakes and ongoing therapy, and facilitate necessary referrals to enhance the health and well-being of the patients. The BCM is also responsible for developing and maintaining relationships with their assigned partnering clinics and providers. The BCM will partner with the Clinical Operations Manager for monthly and quarterly presentations to their assigned clinic leadership.

Reports To: Clinical Operations Manager

Salary Range: Starting at $60,000 annually, DOE licensure and experience, plus performance-based bonuses

Location: The BCM role is a hybrid position that requires duties to be performed both in-person at the clinic in Frisco, Texas and virtually. 

Key Responsibilities:

Patient Engagement:

  • Directly engage with pregnant and postpartum patients, and those who have experienced loss, in OB clinics to actively educate, recruit, and enroll eligible patients in FamilyWell’s perinatal collaborative care program.

  • Communicate with patients through various channels (in-person, text, phone) to facilitate program recruitment, provide reminders, and follow up on care.

  • Aim to achieve and maintain an 80% patient engagement rate in the program.

Enrollment and Eligibility:

  • Review patient eligibility, verify insurance coverage, and complete all necessary enrollment documentation.

  • Ensure accurate and timely completion of all enrollment requirements.

Direct Patient Care:

  • Conduct one-on-one psychological intakes for referred FamilyWell program patients, gathering comprehensive assessments to inform treatment planning.

  • Provide individual tele-therapy to patients requesting regular sessions using FamilyWell’s virtual platform.

Care Coordination and Documentation:

  • Send mental health screenings, reminders, and follow-up on results.

  • Facilitate outside referrals for community-based social services as clinically indicated (e.g., housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

  • Update patient records in Electronic Health Records (EHRs) and communicate care plans to the clinic providers.

  • Maintain a collaborative care registry provided by FamilyWell to track patient follow-up and clinical outcomes.

  • Prepare and submit monthly patient progress reports in the EHR.

Reporting and Collaboration:

  • Compose, prepare, and communicate timely patient and provider responses to questions.

  • Report directly to the Clinical Operations Manager and provide regular updates on patient engagement and clinical outcomes.

  • Collaborate with OB clinic staff to ensure coordinated care and support for patients.

  • Develop and maintain relationships with partnering clinics and clinical teams.

  • Develop and deliver monthly and quarterly presentations to partners. 

Qualifications

Education:

  • Master’s degree in Social Work, Counseling, Psychology, or a related field 

Licensure/Certification

  • Licensed Clinical Social Worker (LMSW or LCSW) 

Skills:

  • Strong understanding of perinatal mental health conditions, treatments, and community resources.

  • Excellent communication, organizational, and problem-solving skills.

  • Ability to engage and educate patients in a compassionate and supportive manner.

  • Proficiency in using EHR systems and maintaining accurate patient records.

  • Proficiency in using Google Suite.
